I, the undersigned, acknowledge and agree that participation in the Hopewell Hawks AAU basketball program involves physical activity and the risk of injury. I voluntarily assume all risks associated with my child's participation, including injury, illness, or property damage. I release and hold harmless Hopewell Hawks, its coaches, staff, and affiliated organizations from any claims or liabilities arising from participation in team activities, practices, games, or events. By signing, I confirm that my child is physically fit and able to participate, and I agree to abide by the program's rules and guidelines.
